It will compare outcomes of VA patients and non- VA patients for 10 high volume medical an surgical diagnoses, adjusting for severity of illness. The study will then identify characteristics of VA patients that are associated with poor outcomes and estimate the prognostic impact of these characteristics. |

Wakefield, Ph.D. | Effectiveness of Home Care Coordination in Managing Medically Complex Patients | This study will evaluate the efficacy of home telehealth in improving outcomes in veterans with co-morbid diabetes and hypertension. It will compare outcomes of patients who receive home telehealth care to outcomes of patients who receive usual care. |
